How to Apply

Please download the four PDF files below. 

Study all of the materials in the application packet and completely fill out each form, submit them along with the $25.00 one-time application fee to the QBIS offices or mail your completed forms to:

If you do not have a printer you will need to go to our ‘Contact Us’ page  and request an application packet. You can also stop by the QBIS offices and pick up a packet from the QBIS front desk.
